Affiliation as a

Contributor

.

To apply for affiliation as a contributor, the interested party must enter the UISALUD online option where he/she must:

• Fill out completely and correctly the data requested in the Pre-affiliation form.

• Fill out completely and correctly the data requested in the health declaration form.

• Submit a photocopy of the ID card enlarged to 150% (scan and upload it in PDF, GIF or JPG format).

• Send a letter of authorization for the discount of contributions addressed to the Human Resources Office of the UIS, according to the plan chosen, prior knowledge of the service provision regulations (scan and upload in PDF, GIF or JPG format).

Basic health plan: Authorization for a discount of four percent (4%) of the mandatory health plan.

Additional health benefits plan: Authorization for an ADDITIONAL discount of two point five percent (2.5%).

Affiliation of

Spouse or permanent partner

.

For spouse or permanent partner affiliation:

• For spouse: Current marriage certificate

• For permanent partner: Public deed before a notary or conciliation deed signed by the permanent partners in a legally constituted Conciliation Center or the Judicial Sentence that accredits the existence of the union.

• Declaration of economic dependence (According to model).

• Photocopy of the citizenship card enlarged to 150%.

• A copy of the divorce certificate, when a new spouse is requested to be affiliated as beneficiary.

• Certification of withdrawal from the previous EPS.

Affiliation of

Children

.

For affiliation of children:

• Civil registry that accredits kinship.

• Declaration of economic dependence (according to model).

• Photocopy of the Citizenship Card or a copy of the password for children over eighteen (18) years old, enlarged in 150.

• Photocopy of the Identity Card, for children between seven (7) and eighteen (18) years old, enlarged in 150.

• A certification issued by the Regional Disability Qualification Board, to prove the quality of the child with permanent disability.

• Adoption Certificate issued by the competent authority, to prove the quality of adopted child.

• Certificate of delivery subscribed by the competent entity in which it accredits the quality of “minor given in legal custody”.

• To prove the condition of student, original Certification issued by the establishment of basic, middle or higher formal education, approved by the Ministry of Education, in which studies with an intensity of at least twenty (20) hours per week are studied, must be presented.

• Certification of withdrawal from the previous EPS.

Affiliation of

Parents

.

Conditions:

• In the absence of the spouse or permanent partner and children, the parents of the contributing member who are not entitled to another health service, are not pensioned and are economically dependent on the contributing member.
• When both spouses or permanent partners, including same sex couples, are contributors to UISALUD, the parents who are economically dependent on one of the spouses or permanent partners and who are not contributors to the Social Security Health System or are not obliged to do so, may be enrolled in the family nucleus.
• When both (2) spouses contribute to UISALUD and have children, one of them may enroll their children and the other may enroll the entitled parents.

Documents required:

• Civil registry proving kinship.

• Declaration of economic dependence (according to model).

• Photocopy of the Citizenship Card of the parents enlarged to 150.

• Civil Registry of Marriage (updated) of the contributors.

• Letter according to model, signed by both contributors.

• Certification of withdrawal from the previous EPS.
